Diagnostic Crossover in Anorexia Nervosa and Bulimia Nervosa: Implications for DSM-V

Abstract: Objective-The Diagnostic and Statistical Manual of Mental Disorders (DSM) is designed primarily as a clinical tool. Yet high rates of diagnostic "crossover" among the anorexia nervosa subtypes and bulimia nervosa may reflect problems with the validity of the current diagnostic schema, thereby limiting its clinical utility. This study was designed to examine diagnostic crossover longitudinally in anorexia nervosa and bulimia nervosa to inform the validity of the DSM-IV-TR eating disorders classification system.… Show more

“…It was decided to investigate the clinical group as a whole because of the evidence of high diagnostic crossover between AN and BN (Eddy et al, 2008), and to ensure sufficient power for the analysis. Results indicated that in the clinical group, DS duration and intensity were significantly associated with BMI.…”

“…It was decided to investigate the clinical group as a whole because of the evidence of high diagnostic crossover between AN and BN (Eddy et al, 2008) In the eating disorders group, the duration of NDS was found to be negatively associated with TAS-20, and NDS intensity was significantly associated with BMI. NDS was not found to be associated with the other clinical measures (see Table 3).…”

“…Patients with AN who rarely binge-eat or purge but maintain a fairly regular pattern of caloric restriction may be classified as having the restricting subtype, whereas those who regularly engage in binge eating and/or compensatory behavior to prevent weight gain will be diagnosed as having the bingeeating/purging subtype [7]. Many of those with the restricting subtype will eventually develop binge eating, with at least one-third of patients crossing over into BN [13]. Crossover to binge eating and BN typically occurs within the first 5 years of the illness [13].…”

Crossover to binge eating and BN typically occurs within the first 5 years of the illness [13]. Women with AN who develop BN are likely to relapse back into AN [13].…”
