Foi determinado o perfil de resistência de bactérias isoladas de diversas afecções em cães e gatos atendidos no Setor de Clinica Cirúrgica de Animais de Companhia do Hospital Veterinário da Universidade Estadual de Londrina. Houve maior frequência de Staphylococcus spp. (27,6%), seguido por Pseudomonas spp. (22,7%) e Escherichia coli (16,6%). Na prova de sensibilidade antimicrobiana pelo método de difusão em ágar houve alta porcentagem de resistência das bactérias isoladas aos principais antibióticos usados no tratamento das infecções do trato urinário, principalmente das bactérias Gram negativas que apresentaram resistência superior a 66% aos antibióticos testados, com exceção da norfloxacina. Nas bactérias isoladas de feridas, apenas a gentamicina e a amicacina demonstraram índices de resistência inferior a 50,0%. Nas bactérias isoladas das afecções otológicas observou-se menor resistência à norfloxacina e maior à neomicina, sendo os menores índices de resistência observados nas bactérias Gram positivas. As bactérias Gram positivas apresentaram maior resistência à ciprofloxacina nos casos ortopédicos, e nas bactérias isoladas das peritonites houve 100% de resistência a diversos antibióticos. Este trabalho ressalta a importância da identificação bacteriana e a realização de antibiogramas para a escolha do agente antimicrobiano apropriado no tratamento das principais afecções atendidas na área de animais de companhia em medicina veterinária.
Pesq. Vet. Bras. 32(12) Spinal cord injury is a common cause of neurological dysfunction in dogs and cats. Lesions in these species are due to various types of accident, which can cause sequelae that impair the patient for life as a pet, or cause life-threatening injury. The main purpose of this work was the accompaniment of animals with spinal cord trauma seen from August 2009 to November 2010 at the Veterinary Hospital of Universidade Estadual de Londrina, studying the epidemiology and etiology, risk factors, spinal cord segments most affected, outcome of conservative or surgical treatment, ratio of time of patient care with recovery, efϐicacy and side effects of neuroprotective drugs, complications, sequels and evolution of patients. During this period we monitored 57 animals (48 dogs and nine cats). We observed a predominance of males (68%) and indoors (79%). The main cause of injury was being hit by a car (66%). The time between injury and ϐirst attendance was less than eight hours in 42% of cases and more than a day in 51%. The spinal segment most affected was the thoracolumbar (52%). Twenty animals were euthanized after the initial attendance due to poor prognosis. The conservative treatment such as rest and/or external immobilization was performed on 29 animals, and a good outcome was seen in 72.4% of this patients: total functional recovery was observed in 17 (58.6%) animals and partial functional recovery was seen in four (13.8%). Four animals did not recover, and four animals died. Surgical treatment was performed in eight patients, and three animals recovered, one patient did not recover and four animals died or were subjected to euthanasia due to complications during intraoperative or postoperative period. ResumoA criptococose é uma micose sistêmica oportunista causada pela levedura Cryptococcus neoformans. A infecção em cães ocorre mais comumente pela inalação de leveduras ou esporos. As alterações neurológicas encontradas são muito semelhantes a outras doenças inflamatórias e infecciosas do sistema nervoso. O objetivo do presente trabalho é relatar três casos de criptococose no sistema nervoso de cães, ressaltando a importância desta doença no diagnóstico diferencial de alterações neurológicas progressivas ou multifocais associadas ou não a alterações sistêmicas. O diagnóstico só pode ser confirmado através de cultura fúngica e/ou visualização do agente após coloração adequada do líquor, ou sorologia, quando disponível. The infection in dogs occurs commonly by inhalation of the airborne or yeast. The neurological signs in dogs are similar to other inflammatory and infectious diseases of the central nervous system. The aim of the present paper is to report three cases of cryptococcosis in the nervous system of dogs, and reinforce the importance of this disease in differential diagnosis of progressive or multifocal alterations in the nervous system whether linked or not to systemic alterations. The diagnosis can only be confirmed through fungal culture and/or the visualization of the agent after adequate staining of the cerebral spinal fluid, or antibody titer when it is available.
ResumoA resistência aos agentes antimicrobianos tornou-se uma das principais preocupações para a saúde humana, mas também está se tornando problema em medicina veterinária. Os antimicrobianos são usados mundialmente para tratamento e profilaxia de doenças infecciosas em animais de produção, equinos e animais de companhia e como promotores de crescimento. Em muitos estudos observou-se que o uso de antimicrobianos em animais contribuiu para o desenvolvimento de resistência antimicrobiana em humanos, pois muitas classes de antimicrobianos usadas em animais também são empregadas em seres humanos para tratar doenças graves, ocorrendo falha terapêutica, devido à transferência de bactérias resistentes pela cadeia alimentar. Alguns autores questionam se este risco para a saúde pública originouse somente do uso de antimicrobianos em animais, visto que estas substâncias são usadas na agricultura e também são encontradas naturalmente no meio ambiente. Assim a resistência bacteriana pode se desenvolver mesmo sem o uso de antimicrobianos. Logo, neste artigo, busca-se revisar o papel dos animais na transmissão de bactérias resistentes aos seres humanos e vice-versa questionando ainda a existência de outras fontes de resistência. Antimicrobial agents are used worldwide in veterinary practice for therapy and prophylaxis of infectious diseases in food, horses and companion animals and for growth promotion in food animals. Many studies have shown that this kind of antimicrobial use in animals contributed to the selection of antimicrobial resistance, because many classes of antibiotics used in animals are used to treat serious diseases in humans, causing therapeutic failure, because of transference of resistant bacteria through food chain. Some authors question if this hazard to public health originated only from the use of antibiotic in animals, since these substances are also used in agriculture and are naturally found in the environment. So the antimicrobial resistance may develop even without the use of antibiotics. Therefore, the present article reviews the role of animals in the transmission of resistant bacteria to humans and vice versa, and questions about the existence of others sources of resistance.
RESUMOAvaliaram-se os resultados clínicos após realização de cirurgia descompressiva em 45 cães com doença do disco intervertebral cervical ou toracolombar. Após a cirurgia, 35 cães (77,8%) recuperaram-se totalmente, oito (17,8%) parcialmente e dois (4,4%) não apresentaram alteração do quadro inicial. Em oito cães com paraplegia e perda da sensibilidade dolorosa profunda houve completa melhora do quadro clínico, com recuperação total em 62,5% dos casos. Em quatro cães com tetraparesia, a cirurgia foi eficaz. A cirurgia descompressiva (slot cervical e hemilaminectomia toracolombar), com a retirada do material do disco do interior do canal vertebral, foi uma forma efetiva de gerar melhora do quadro funcional. O trauma medular é uma das entidades neurológicas mais freqüentes e mais graves na prática clínica. A lesão medular aguda inicia uma seqüência de eventos vasculares, bioquímicos e inflamatórios que resultam no desenvolvimento de lesões teciduais secundárias, levando à destruição progressiva do tecido neuronal com conseqüências desastrosas e freqüentemente irreversíveis às funções motora e sensorial do animal. Esta afecção deve ser considerada emergencial, visto que a intervenção rápida e adequada em intervalo de tempo apropriado, pode limitar a extensão dos danos ao tecido neuronal, favorecendo assim a recuperação neurológica do paciente. Em geral, no tratamento da lesão medular aguda são utilizados agentes neuroprotetores, visando o controle das lesões secundárias, associados ou não à cirurgia para descompressão e estabilização da coluna vertebral. A compressão medular crônica pode ser decorrente de processos que se desenvolvem gradativamente, mas também pode se referir a seqüelas decorrentes dos efeitos do trauma, meses a anos após a ocorrência da lesão. O objetivo deste trabalho é revisar a fisiopatologia do trauma medular, o tratamento médico disponível, as opções para o futuro e as controvérsias existentes sobre o uso de alguns fármacos.
